According to the 5.11 website, this item comes in 3 sizes:

Regular

2XL+

4X-Large

I'm 6'3", 200lbs, not muscular, 25% body fat and I generally wear an XL size t-shirt.

The 'regular' works for me.

The vest allows for a lot of size adjustment.

The next size up would only fit me if I were in a very cold climate and was wearing lots of clothes underneath it.

This vest is awesome. I purchased the black 2XL, I am 6' 3" and weigh 285 pounds, I have a 54" chest and a 50" waist, it adjusts as snug as you need and allows for complete mobility. As soon as I took it out of the box I let my 7 month old son play with it (he tears magazines to shreds in seconds) to see how well put together it is, and it is tough with no visible wear and tear after being put through it's paces. The zipper is extremely sturdy, all of the stitching looks good and feels sturdy, the velcro works great, and I've already attached several MOLLE compatible pouches to it which fit great, as do carabiners. I shopped around for a quality vest for quite some time and I am so satisfied with the result. Thank you 5.11.

Fits well but can adjust to your body (top and sides). I like the fact that you can attach items with the MOLLE style hookup anywhere on the vest. I also ordered the Blackhawk speed strips because I spent 10 min getting just the left side of an AR-15 mag case attached. After using the speed strips, I put on the rest of my articles (total of 4 in just 20 min).

Been looking for a strong, solid M.O.L.L.E. vest for a couple of years now, but I was hesitant to get any of them because conflicting size reviews, quality issues, etc.

This one is excellent. M.O.L.L.E. options from front to back, giving nearly unlimited options.

My only issue involves the velcro shoulders. Yes, they are on the under side of the vest, so dirt and grime won't wear away at the velcro, but it took a bit of adjusting for my height, and eventually I was afraid I would wear the velcro out doing that alone. It still raises above my shoulders a little and it's stiff, but it just requires some breaking-in.

I might have been better off getting the XL, instead of the XXL, but they're all adjustable from all sides, so it should work for you as well.
